APPLICATION PROCESS
To apply to the Teen Challenge Farm, please follow the process below. Applicant must fill out the application form himself. Eligibility requirements are a life-controlling drug or alcohol problem, a sincere desire for help and a commitment to the 12 month program.

1A. Interview and Application
- Attend an on-site application interview. Please call the Intake Coordinator at 1-888-417-7777, ext. 1270 to set up an appointment.
- If you live more than 3 hours away or are incarcerated, fill out an application form using one of these two options and a phone interview will be arranged:
- ONLINE (using our secure internet server)
- PDF (download, print and mail or fax) fax/phone/address
1B. $100 Application Fee (certified cheque, money order, cash, VISA, MasterCard)
- This fee can be paid when you come in for the on-site application interview
- If you live more than 3 hours away or are incarcerated, please use one of these two options:
- On-line (using our secure internet server)
- Mail to or phone (be sure to include name of applicant) fax/phone/address
2. Approval requirements
Applicants are required to provide:
- Police Criminal Record Check (contact your local police)
- Pre-Entrance Medical Exam (to be filled out by your doctor)
- Court dates or probation
